Outcome: Each participant is actively involved in the development of their support plans. Support plans reflect participant needs, requirements, preferences, strengths and goals, and are regularly reviewed.
Golden Wattle Care Perth Pty Ltd will develop support plans in collaboration with each participant and, with consent, their support network. The director advised that information such as participant goals, preferences, strengths, medical history, key contacts, and daily support needs will be gathered during intake and used to create a plan that reflects the individual.
Risk assessments will be completed alongside the support plan, considering the participant’s reliance on supports and the impact if services are disrupted. Strategies will be put in place to manage identified risks, and these will be reviewed regularly with the participant to ensure they remain effective and appropriate.
Support plans will be reviewed at least annually or earlier if there are changes in the participant’s needs, circumstances, or progress. Where outcomes are not being met, the plan will be updated in consultation with the participant to better align with their goals.
Each plan will be provided in a format that the participant can understand and will be accessible to both the participant and workers delivering supports. With consent, relevant information will be shared with family members, other providers, and services to support coordinated care.
Golden Wattle Care Perth Pty Ltd ensures support plans will also include arrangements for preventative health supports, such as access to medical and allied health services, as well as clear guidance for emergency and disaster situations to maintain participant safety and continuity of supports. Workers will be supported to understand and follow each participant’s plan in practice.
