Each participant accesses responsive, timely, competent and appropriate supports to meet their needs, desired outcomes and goals.
Supports are provided based on the least intrusive options, in accordance with contemporary evidence-informed practices that meet participant needs and help achieve desired outcomes.
Where a participant has specific needs which require monitoring and/or daily support, workers are appropriately trained and understand the participant’s needs and preference.
Golden Wattle Care Perth Pty Ltd will deliver supports in a way that is flexible and responsive to each participant, using the least intrusive approaches to support independence and achieve desired outcomes. Supports will be adjusted based on the participant’s needs, preferences, and feedback, with regular communication to ensure services remain suitable and effective.
With participant consent, the provider will work with other services, including healthcare and allied health professionals, to share relevant information, manage risks, and ensure supports are coordinated and effective.
The director advised that participants will be involved in choosing their support workers where possible, including preferences such as gender for personal care. These preferences will be discussed during intake and followed in service delivery.
Golden Wattle Care Perth Pty Ltd ensures where participants require monitoring or higher levels of support, workers will be trained to understand the participant’s needs and provide consistent care. Supports will be guided by the participant’s plan to ensure they remain appropriate and responsive.
