Outcome: Each participant accesses the most appropriate supports that meet their needs, goals and preferences

The supports available, and any access / entry criteria (including any associated costs) are clearly defined and documented. This information is communicated to each participant using the language, mode of communication and terms that the participant is most likely to understand.

Reasonable adjustments to the support delivery environment are made and 2 monitored to ensure it is fit for purpose and each participant’s health, privacy, dignity, quality of life and independence is supported.

Each participant is supported to understand under what circumstances supports can be withdrawn. Access to supports required by the participant will not be withdrawn or denied solely on the basis of a dignity of risk choice that has been made by the participant.

Supports will be adjusted to suit each participant’s needs, preferences, and environment. The provider will monitor how supports are delivered and make changes where required to ensure the participant’s health, privacy, dignity, independence, and overall wellbeing are maintained. Feedback from participants and their support networks will be used to guide these adjustments.

Golden Wattle Care Perth Pty Ltd ensures participants will be informed of the circumstances under which supports may be changed or withdrawn, including any notice periods. The provider will ensure that access to supports is not withdrawn solely because a participant chooses to take reasonable risks. Where participants wish to make changes or discontinue services, they will be supported to do so in a planned and respectful manner.
